USE THIS FORM IF......ALL OF THE TEAMS IN YOUR LEAGUE, DIVISION OR GROUP (MINIMUM OF 5 TEAMS) IS PURCHASING ASA INSURANCE.

This form is for Softball teams in groups of a minimum of 5 or more teams, or in divisions or leagues to purchase insurance for all of their teams. It is based on your team being part of a group consisting of at least five teams. Liability coverage includes each opponent you play (regardless of whether or not your opponent is in your league or has any insurance), the field owner, your sponsors, players, coaches, passersby and spectators as well as personal property on or near the field with $5 million of liability insurance with a $250 or $500 deductible, depending on your policy. Your players will ALWAYS be covered with $250,000 of injury insurance while playing on any ASA or USA softball team throughout the nation for the entire calendar year.

This form is for groups of softball teams (minimum of 5 per group), divisions or leagues in which all of the teams will be covered by ASA insurance. Your team's coverage is based on being in a group of 5 or more teams. Each player on teams in the group will be covered by accident injury insurance, plus your team and each of the opponents you play will be covered by liability insurance.
